Some life insurance carriers have very favorable underwriting guidelines for applicants with a history of prostate cancer.

For example, one carrier may consider an applicant age 60 or older at diagnosis, with a Gleason score of 6 or lower, no evidence of invasion, a pre-treatment PSA below 10, an undetectable post-treatment PSA, and at least 12 months since surgery for preferred rates—assuming the rest of the medical history and underwriting requirements are favorable.

Even applicants with a Gleason score of 7 may have several carrier options available. Certain treatment histories, including seed implants and radiation, may also fit within favorable underwriting guidelines depending on the carrier and individual circumstances.
